Glasgow Royal Infirmary opened in December 1794 on the most historic ground in the city — the site of the demolished Bishop’s Castle, hard by the cathedral — in a handsome domed building designed by Robert Adam and completed by his brother James. The site was the point: the growing industrial town’s sick poor were concentrated in the old High Street quarter, and the Royal was built, by subscription and charity, to serve them without charge.
Within its walls, in the 1860s, occurred one of the turning points of human history. Joseph Lister, the Royal’s professor of surgery, confronted the era’s surgical scandal — operations succeeded, then patients died in days of ward infections — and, applying Pasteur’s germ theory, began in 1865 treating wounds and instruments with carbolic acid. Mortality in his wards collapsed; his 1867 papers announced antiseptic surgery to the world; and every sterile theatre on earth descends from what Lister proved in the Glasgow Royal. His pupil-successor William Macewen went further here, pioneering aseptic technique and performing some of the first successful brain surgery anywhere.
The Royal’s firsts kept coming. In 1896, within months of Röntgen’s discovery, the physician John Macintyre established at the Royal the world’s first hospital radiology department, X-raying kidney stones and a frog’s dancing skeleton with equal Victorian relish; and in nursing, matron Rebecca Strong created in 1893 the first systematic preliminary training school for nurses, a model adopted worldwide.
The beloved Adam building did not survive its own success: needing scale, the infirmary rebuilt itself in 1907–14 to James Miller’s massive baroque designs — a demolition of Adam’s work protested then and regretted since — and the Royal has kept growing ever since, its later towers looming in every photograph of the cathedral precinct, to the periodic despair of conservationists and the daily relief of the East End.
Two hundred and thirty years on it remains a great teaching hospital on its original ground — the rare institution that is simultaneously a working A&E and a world heritage of medicine: the place where surgery stopped killing its own patients.
